Rapid Maxillary Expansion (RME) Appliance: What It Is and How It Works
This appliance is commonly used in children and teenagers to widen the upper jaw (maxilla) when it is too narrow. A narrow jaw can cause problems like crowding of teeth, crossbites, and difficulty breathing through the nose. Usually diagnosis is done using an OPG radiograph.

How Does the RME Appliance Work?
The RME appliance is a custom-made device that fits against the roof of the mouth and is attached to the upper molars using metal bands or acrylic supports. In the center of the appliance, there is a small screw (expansion screw) that is used to gradually widen the upper jaw.
Activation Process:
- Daily Adjustments – A special key is provided to turn the expansion screw. The orthodontist will instruct you on how to do this at home.
- Turning the Screw – Usually, the screw is turned once or twice a day, depending on the treatment plan.
- Gentle Pressure – Each turn applies light pressure to separate the bones of the upper jaw.
- Visible Space – Within a few days or weeks, a small gap may appear between the two front teeth. This is normal and shows that the jaw is widening.
- Stabilization Period – After achieving the desired expansion, the appliance remains in place for a few months to allow new bone to form and stabilize the expanded jaw.
What to Expect?
- Mild pressure or tightness for a few minutes after turning the screw.
- Temporary changes in speech as the tongue adapts to the appliance.
- Possible gap formation between the front teeth, which can later be closed with braces.
Why is RME Important?
Rapid maxillary expansion helps create enough space for permanent teeth to align properly, reduces the risk of crowding, and can even improve breathing by opening up the airway. It is most effective in growing children and teenagers before the bones fully harden.
